J.D. and Turk decide to forgo their beloved steak night, a cherished ritual they've upheld for ten years, in order to honor the wishes of a terminally ill patient. At the same time, Dr. Maddox, the newly appointed Chief of Medicine, starts revealing her true nature and intentions, causing ripples in the hospital's dynamics.
